How do I sign up for my membership?
Talk to anyone at the front desk or call our office and we will get you enrolled.
What can I expect at my first visit?
We are excited to have you as our patient! On your first visit, pay the one-time enrollment fee and first monthly payment and receive the included benefits of the plan you choose.
What is a Consistent Care™ visit?
A Consistent Care™ visit includes your preventative regular care of a doctor exam, x-rays (as needed) and a professional cleaning. Depending on the plan you choose, it may also include additional benefits such as bleaching, etc.). This saves you money when work is needed.
Is there a waiting period for benefits and services?
No! There are no limitations or waiting periods for current or future dental needs.
What should I do if I have periodontal disease?
We have a Perio Plan specifically designed for you. It includes your regular perio maintenance visits as well as additional discounts on specific perio services. During your initial visit, we will let you know if these services are needed.
For Premier Members Only
To encourage you to use your included preventative benefits, Premier members will receive a $100 reward after each Consistent Care™ visit. Rewards may be used for services you need (crown, filling, veneers, root canal, etc.) and don’t expire as long as your membership remains active.
Individual rewards may be combined with other family members to maximize savings. Take advantage of having your entire family on the plan by using everyone’s rewards on a single procedure.
The more often you come in and use your included preventative benefits the more you can earn!
